Provide individual, group, conjoint or family psychotherapy and support services to clients. Conduct clinical assessments, develop diagnoses, and create person/family centered treatment plans. Monitor progress and coordinate behavioral health services, community outreach, and referrals.
Meet home-based service requirements with minimum 6 hours per family per month, including at least 4 face-to-face hours. Ensure caseload compliance with up-to-date CAFAS/PECFAS assessments. Maintain electronic and paper charts with progress notes, treatment plans, and client rights documents.

Requirements
- Masters Degree in Social Work or Counseling
- Licensure in your field (LLMSW, LMSW, LLPC, LPC, etc)
- At least one year experience providing behavioral health services to youth
- A valid Michigan Drivers License
Responsibilities
- Provides individual, group, conjoint or family psychotherapy and support services to children with severe emotional disturbance and their family
- Conducts clinical assessments, develops diagnoses, and participates in developing person/family centered treatment plans with client families
- Monitors client progress and coordinates appropriate behavioral health and other needed services including community coordination, outreach and referrals
- Meets all requirements for home-based hours (minimum 6 hours per family per month including at least 4 hours face-to-face)
- Ensures full compliance of clients on caseload have up-to-date/current CAFAS/PECFAS on ongoing basis
- Maintains an electronic and paper chart which includes required documentation of services provided, progress notes, treatment plans, periodic reviews and client rights documents
- Participates in individual supervision and team meetings to review progress, identify barriers and develop strategies for better assisting families
