Your North Star is to provide direct patient care and navigation with evidence-based perinatal mental health support focused on awareness, education, and prevention. Deliver one-on-one virtual visits, screenings, psychosocial assessments, and individualized care plans for pregnant and postpartum patients. Address barriers, reduce care gaps, and triage urgent concerns while building rapport and wellness goals.
Report to the Director of Therapy within a clinical team supporting whole-person emotional, mental, and behavioral care. Collaborate with multidisciplinary teams and support services to meet patient and family needs. Promote diversity, equity, and inclusion to reduce disparities in perinatal care access.

Requirements
- Independently licensed as an LCSW, LMFT, LPC or LMHC for at least 5 years
- Completed the NCMHCE and/or NCE
- Have at least 120 hours of clinical supervision completed
- Have 5-8 years of experience providing therapy services in a hospital and/or healthcare practice
- Have experience with perinatal mental health care, including perinatal mood and anxiety disorders, substance abuse disorders, trauma-informed care
- Enthusiastic for promotion of diversity, equity, and inclusion in all aspects of programs
- Possess strong organizational skills and ability to prioritize and follow through on multiple projects
- Facile with email, spreadsheets, EHRs, and other commonly used software
Responsibilities
- Provide one-on-one virtual care visits for Pomelo patients
- Provide behavioral health and health related social needs screenings with diagnostic, therapy, and behavioral health interventions
- Perform ongoing psychosocial assessment of patients, providing reassurance and building rapport
- Establish individualized care plans to meet specific health needs based on evidence-based standards
- Develop wellness goals by determining motivating factors for best possible health outcomes
- Identify and address barriers identified by patients
- Reduce care gaps like missed appointments and medication management through frequent patient engagements
- Triage patients with urgent screening results to appropriate virtual and in-person emergency services
